1. Application & Rules Agreement
2. Eviction Waiver & Indemnification
3. Information Release
4. Personal History & Information

STEP 2. Scan or photograph the completed forms and e-mail them as attachments to thejoshhouse@gmail.com. In the e-mail, tell us how you heard about us.

STEP 3. If you are accepted, we will contact you for payment, issue a rules handbook, and arrange a start date.
